VHS is looking for a qualified Physician - OB/GYN.

- Description: Summary: St. Elizabeth is looking for OB coverage
for locums coverage for June 2025. Ongoing dates will be TBD as the
facility completes their schedules. Site will put a preference on
locums who are local to the facility due to some stints being 1
day. Rate Caps for Submission:Malpractice Must be included in
rates.Orientation 200 - half the hourly/callback rate kicks in
after 8 hours. Per hour of orientation or training beyond 8 hours
On Call Daily 3300 - Per 24 hours to include 4 hours gratis24 Hour
Holiday Call Rate - 4950 - Per 24 hours to include 4 hours
gratisCall Back 400 - Per hour of patient contact hours after
gratis hours are completed gratis prorate to MSA terms. Holiday
Call Back 600 - Per holiday hour of patient contact hours after
gratis hours are completed gratis prorate to MSA terms. Average
Worked Callback Hours: 5 hoursSchedule/Availability

virtual EMR training to attend the training.EMR Virtual training -
Only able to attend if Modules are completed before training.
Virtual EMR training must be scheduled prior to the providers first
day to avoid the provider missing their first day. All candidates
must complete the following EMR questionnaire and detail it in the
notes section of the candidates submission at time of
presentation.What are the # of years of epic experience they
have?When was the last time MM/YYYY that they used Epic?What is the
name of the last facility the provider used EPIC?Can you provide me
with a photo large enough to add to a badge and not become

example a dietary restriction necessitating specific lodging
