Job Description:
Stellar Living, a recognized leader in senior care, seeks an
experienced MDS Coordinator to join our team. As part of our
commitment to enhancing the lives of seniors, the MDS Coordinator
will play a critical role in our interdisciplinary team, overseeing
the Assessment and Care Planning process in compliance with federal
and state regulations. This position requires a detail-oriented
professional, adept at multi-disciplinary coordination, timely
documentation, and adapting care assessments based on evolving
health needs. Coordinate and ensure accurate and timely completion
of the Minimum Data Set (MDS), Care Area Assessments (CAAs), and
Care Plans. Collaborate with nursing staff, physicians, and other
healthcare professionals to gather patient information and
treatment plans. Facilitate and participate in care plan
conferences with residents, family members, and team members.
Monitor and interpret patient outcomes, adjusting care plans as
necessary to meet federal and state compliance requirements. Train
and support nursing staff in MDS processes and updates in
compliance regulations. Audit and validate data to ensure accuracy
for quality measures and reimbursement. Proficiency with MDS 3.0
Strong analytical skills Excellent communication and interpersonal
skills Detailed knowledge of healthcare regulations and compliance
Ability to lead and train staff Medical Insurance Dental Insurance
Vision Insurance 401k Valid RN license required Certified MDS
Coordinator (CMDSC) preferred
